Position Highlights: Providers deliver comprehensive primary care services in collaboration with physicians, advanced practice providers, and interdisciplinary care teams. Responsibilities include patient assessment, diagnosis, treatment planning, preventive care, and population health management.
Key Responsibilities:
- Perform clinical assessments and manage acute and chronic conditions within scope of practice
- Collaborate with physicians, NPs, PAs, and nursing staff
- Develop and implement patient-centered care and education plans
- Maintain accurate and timely chart documentation
- Support preventive care, discharge planning, and patient self-management
- Participate in care coordination, team huddles, and quality improvement initiatives
Qualifications:
- Graduate of an accredited Nurse Practitioner or Physician Assistant program
- Active Washington or California license (as applicable)
- Prescriptive authority and required certifications
- Previous clinical experience preferred
- Experience with underserved populations or community health (especially for CA roles) preferred
Compensation & Benefits:
- Competitive salary (California NP roles: $131,000–$170,000 annually)
